How does dental insurance work? And why is it soooo confusing!

Prior to working as an oral surgeon, I had no idea what insurance really did. I knew that when I was younger I was covered by my parent's insurance, and in school I was covered through my school plan, but I really didn't know what any of that meant. Sometimes I got a small bill compared to what I was charged, and sometimes I got the full bill of what I was charged, what gives! And explaining in-network, out-of-network, estimate of benefits...?
